Job summary

The Advanced Clinical Practitioner (ACP) OR Advanced Nurse Practitioner (ANP) focusing on eConsultations is integral to enhancing access to healthcare through digital means. This role involves managing electronic consultations (eConsultations), providing high-level clinical assessments, advice, and treatment recommendations primarily through online platforms and telephone appointments. The ACP/ANP will work under the guidance of and in collaboration with, other healthcare professionals to deliver timely and effective patient care remotely.

Key responsibilities include conducting thorough clinical assessments via eConsultations and telephone calls, diagnosing conditions, prescribing medications or other treatments, and referring patients to other services when necessary. The role demands a high degree of clinical expertise, excellent judgement, and proficiency in using digital health tools to manage patient care efficiently.

The ACP/ANP will also play a critical role in the development and implementation of eConsultation protocols and guidelines, ensuring they align with national standards and best practices. This position requires strong communication skills, the ability to handle complex information, and a commitment to providing patient-centred care in a virtual environment.

Main duties of the job

  • The completion of E-consultation clinics across the Fuller and Forbes Practices.
  • Engage with patient requests using our e-consultation service to provide clinical assessment and diagnose health conditions based on the information provided by the patient.
  • Take part in clinical meeting alongside other remote clinical staff to provide guidance and develop protocols for effectively managing e-consultations.
  • Completion of self and practice generated admin in order to provide an efficient continuity of care.
  • Provide follow up telpehone calls to patients should they require this following the information submitted within the e-consultation.

Gateshead Healthcare is run by the Fuller and Forbes partnership consisting of Dr Mark Fuller (GP Partner) and Mr Methven Forbes (Managing Partner). The 4 surgeries, Crawcrook, Blaydon, Rowlands Gill and Grange Roadprovide care to over 20,000 patients across the 4 sites. The patient population has care provided by a team of dedicated GPs, ANPs, Practice Nurses, healthcare assistants and PCN staff.

Job responsibilities

EConsultation Management:

Clinical Assessments via eConsultations: Conduct comprehensive clinical assessments of patients using eConsultation tools. Evaluate patient symptoms, medical history, and any other relevant information provided through the digital platform.

Diagnosis and Treatment Planning: Utilise clinical expertise to diagnose health conditions based on eConsultations. Prescribe medications, suggest lifestyle changes, or recommend further diagnostic tests as appropriate.

Referral Coordination: Refer patients to other healthcare services when more detailed investigations or specialist care is needed, ensuring a seamless transition from virtual to in-person care.

Protocol Development and Implementation:

eConsultation Protocols: Develop and implement protocols for managing eConsultations, including guidelines for patient interaction, assessment procedures, and follow-up care.

Quality Standards: Ensure that eConsultation practices meet national clinical standards and best practices to provide high-quality, patient-centred care.

Training and Support: Provide training to other healthcare professionals on the effective use of eConsultation systems, enhancing the practice's overall capability to manage patient care digitally.

Patient Communication and Education:

Patient Guidance: Instruct patients on how to use eConsultation services effectively, including how to submit information and what to expect during and after the consultation.

Health Education: Deliver targeted health education and advice during eConsultations, empowering patients to manage their health proactively.

Follow-Up Care: Arrange or advise on appropriate follow-up care, including further consultations, tests, or in-person visits, ensuring comprehensive patient management.

Record Keeping and Data Management:

Documentation: Maintain detailed records of all eConsultations, including patient communications, assessments, interventions, and outcomes.

Data Security and Privacy: Adhere to legal and regulatory requirements regarding data security and patient privacy while managing eConsultation records.

Outcome Monitoring: Track and analyse the outcomes of eConsultation services to identify trends, measure effectiveness, and guide improvements in the service.

Collaboration and Professional Development:

Multidisciplinary Team Collaboration: Work collaboratively with a multidisciplinary team including GPs, nurses, and other specialists to integrate eConsultation findings into overall patient care plans.

Continuous Learning: Keep abreast of developments in digital health and eConsultation technologies, as well as advancements in clinical practice that impact patient assessment and care.

Professional Contributions: Participate in professional forums and contribute to scholarly articles or presentations on the subject of eConsultations and digital patient management as it evolves.

Qualifications

Essential
  • Educational Background: Masters degree in Advanced Practice. Additional qualifications in digital health or telemedicine are highly advantageous. Experience: Extensive clinical experience as an Advanced Clinical Practitioner, including significant roles in managing patient care via digital platforms. Proven track record in conducting e-consultations, with strong diagnostic skills and the ability to provide comprehensive care remotely. Clinical Skills: Advanced assessment and diagnostic skills, capable of conducting thorough clinical evaluations and making informed treatment decisions. Proficiency in prescribing and managing medications, with a clear understanding of pharmacodynamics and pharmacokinetics relevant to a range of conditions. Technical Proficiency: Skilled in the use of digital health tools and platforms for e-consultations, including patient management systems and telemedicine technologies. Familiarity with electronic health records (EHR), ensuring accurate and secure handling of patient information. Communication Skills: Exceptional communication abilities, both oral and written, necessary for effective patient consultations and interdisciplinary collaboration. Strong interpersonal skills, with the ability to manage sensitive situations and maintain patient confidentiality.
