Advanced Clinical Practitioner / Triage Lead

Job summary

We are looking for an experienced Advanced Clinical Practitioner to join the team, acting primarily as a Triage Lead in our Appointment Hub.

We are looking for someone wanting a part-time role between 22-28 hours per week, with the majority of hours worked in the mornings. We will work flexibly with the best candidate to agree a mutually suitable weekly rota pattern. However, it is likely to include either a Monday to a Friday to support the busiest days in practice.

We operate a digital front door for all requests for GP care supporting patients to submit via online or automated telephony or our own Reception team. Requests open at 7:30am and the Hub operates until safe clinical capacity has been reached.

The Hub function is to assess and give patients the right contact, first time with the most appropriate person or service, using a collaborative approach, to deliver the best care within the resources available. The Hub team consists of one clinician and two trained care navigators and works closely with the oncall GP that day.

We have seen a reduction in the early morning pressures and an increase in appropriately booked appointments within the practice, along with an improvement in staff well-being.

Main duties of the job

The Triage Lead is supported by a trained team to assess and triage the daily requests for GP care, via an online system. The clinician's role is to identify and prioritise those requiring same day care or offer routine non-time critical care. The team also refers to other services and gives additional support to those patients who find digital access more difficult.

The Triage Lead works collaboratively with the oncall GP each day and is supported by administrative and management resource. The smooth operation of the Appointment Hub is a collective repsonsibilty.

The Triage Lead also actions up to 10 "quick wins" where patient contact is minimal and a full GP appointment can be saved in the first instance.

The Appointment Hub closes when the practice has reached safe capacity for the day - usually around lunch time.

The clinical team are supported by a talented team of receptionists and administrators led by a Patient Services Manager, an Operations Manager and a Business Manager.

Act as the clinical lead in the Appointment Hub, assessing and triaging requests for primary care, identifying those in need of same day appointments and those who can be seen by other services.

Practise in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice

Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information

Person Specification

Experience

Essential
  • First Level registered Nurse (NMC), HCPC professional/ Pharmacist
  • Registered on their appropriate professional register
  • MSc or equivalent in Advanced practice as detailed in the HEE document titled Multi-professional framework for advanced clinical practice in England
  • Qualified independent prescriber (V300)
Desirable
  • Ability to triage and / or experience of acute care settings.
