Digital and Transformation Lead

Job summary

This is an exciting opportunity to join our team by driving a new vision to help develop and transform new and existing services across the Primary Care Network.

The role will require experience in business / service improvement with expertise in transformational change and a good understanding of general practice systems and processes, including broad familiarity with general practice digital systems and how they can be used to deliver improvement.

The post holder will plan, coordinate, and directly support delivery of transformation activity in general practice within the PCN with a view to improving patient access, staff and patient satisfaction, and the efficiency and sustainability of general practice services. Working closely with staff at practice and PCN level to understand current service models, they will identify challenges, agree priorities for improvement and then support these to be delivered.

This is a practice-based role with an element of hybrid working, subject to business need.

Core responsibilities

The Digital and Transformation Lead supports increased access to care for patients, by

  • supporting the adoption of new technology and other initiatives to improve the care offer, and
  • enabling PCN staff to work more effectively to support the sustainability of general practice services.

The post-holder is expected to deliver a combination of the following responsibilities:

  • Improve adoption of new technology to enhance patient access and experience and increase PCN productivity.
  • Build relationships and facilitate collaboration between practices to support the delivery of care to each others patients (for example to delivery of enhanced access or other hub type working arrangements)
  • Review and improve the PCNs digital maturity.
  • Use data, and improve data quality, to:

o Understand demand, capacity and activity and drive improvements in:

- Patient experience of access

- Operational efficiency

- Staff experience at work.

o Support population health management, improvements in care quality and PCNoperational efficiency.

o Understand the type and intensity of support needs of their PCN and coordinate thissupport, including through OD programmes.

o Support the effective adoption of national and local initiatives, including integratedworking at neighbourhood and place level to improve access to services for patients; and

o Ensure that digital and operational PCN transformation is embedded in, and aligned with,ICS and national strategies.

Additional Responsibilities

  • Have responsibility for the appointments system and analyse/manage appointment data for ARRS staff.
  • Understand and use data to drive improvement and assess impacts. This will include providing returns to ICS/NHS, and liaise and support practices with PCN data and use data to track and assess impact of changes made to operational processes.
  • Liaise with practices and ICB/NHS to coordinate and complete returns including providing appropriate support to practices to achieve accurate and complete data collection.
  • Drive the planning and prioritisation of changes that will improve delivery of care and improve patient and staff experience in the practices within the PCN.
  • Build relationships across the PCN to develop consensus for improvements and to ensure business and clinical staff are fully engaged throughout the process of selection, implementation and deployment of change.
  • Develop and manage an overall transformation plan for the PCN, including identifying interdependencies, managing risks, considering the potential impacts on the wider organisation, and determining resource requirements.
  • Link with the ICS and other partners to gain buy-in for transformation and to support alignment with broader strategy and improvement approaches.
  • Support staff to identify and participate in relevant training and/or professional development opportunities that will support achievement of agreed improvements.
  • Responsibility for developing and supporting delivery of transformational projects across the PCN, this will include communication and engagement across internal and external stakeholders.
  • Ensure that changes are aligned with agreed clinical priorities and have ownership and engagement from clinical leaders and staff.
  • Support practices to communicate changes and new processes effectively and consistently to patients.
  • Lead or participate in relevant working groups, project groups, service meetings and other forums across the PCN (and ICS) to agree changes, share learning, discuss challenges, and resolves issues.
  • Identify and make progress with transformational change, by understanding and interpreting national and system-wide policy developments and initiatives to local circumstances and contexts.
  • Any other duties as may be requested by the Clinical Lead or Operations Director of the PCN.
