PCN Transformation Lead

Job summary

To spearhead a new vision and assist in creating and transforming both new and current services within the PCN, we are seeking to hire a Transformation Lead.

The role forms an integral part of the leadership team working alongside the PCN Clinical Director, Board and Senior Management Team to deliver the PCN strategy, identifying and implementing service transformation and innovation. The Transformation Lead will be responsible for managing agreed projects with a focus on developing strong relationships and partnerships across the healthcare system.

With an external focus, the job will actively seek out development opportunities to support the PCN's resilience. Someone with energy, drive, and a desire to improve patient outcomes would be a good fit for this fascinating profession.

Main duties of the job

The role of the PCN Digital and Transformation Lead is to support increased access to care for patients across our large fully merged PCN. You will lead the adoption and/or optimisation of new technology and other initiatives to improve the care offer and enabling PCN staff to work more effectively to support the sustainability of general practice services.

You will support the Clinical Director and engage with a wide range of stakeholders, utilising population health data to transform primary care services and collaborative approaches to service development and delivery. Developing strong partnerships and actively supporting integrated neighbourhood ways of working.

You will be working on the implementation of identified projects, as agreed by the Clinical Director and Board, supporting the delivery of the NHS Long Term Plan and Network Contract DES and delivering local objectives and priorities.

The role will require good analytical skills and numeracy, knowledge of the NHS and Primary Care.

Job responsibilities

  • To support the PCN Clinical Director and Board in the delivery of the PCN Contract DES
  • Lead on transformation and innovation of services and PCN projects
  • Attend cross organisation meetings
  • Deliver projects to support the PCN and improvements to health outcomes for the patient population
  • Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients
  • Ensure that digital and operational PCN transformation is embedded in and aligned with national strategies
  • Identify and make progress with transformational change by understanding and interpreting developments and initiatives
  • To be committed to on-going professional development
  • To work proactively within the team and ensure integrated multidisciplinary working is successful
  • Provide feedback to PCN leadership and to member practices on progress of current initiatives, capacity for change, progress and issues so that plans can be adapted in response

Person Specification

Experience

Essential
  • Experience of successfully delivering change and improvement programmes in a patient (or customer) facing environment.
  • A track record of communicating and engaging with a wide range of staff and stakeholders; building excellent and trusted relationships.
  • Ability to work and make decisions autonomously, and to identify and act on challenges to move delivery forward, potentially working to tight timescales.
  • Excellent communication skills and an ability to engage successfully with a wide range of people at all levels within an organisation.
Desirable
  • Experience of working in general practice with a good knowledge of general practice systems, processes, and procedures.
