Foundation Consultant Practitioner in Frailty

Job summary

We are seeking to appoint an experienced, dynamic and hands-on Foundation Consultant Practitioner in Frailty. The successful post holder will be responsible for driving forward and shaping the strategic, clinical and professional leadership of the frailty model of care within the South West Division of Southern Health and growing our 'One Team' approach to our Urgent Community Response.

The role is focussed on the delivery of a high quality, evidenced based bespoke programme of care within a contemporary service framework. The Team provides a unique model of care, supporting acutely unwell adults who are experiencing a sudden decompensation in physical, psychological or social wellbeing. Our primary aim is to provide first class health care giving our service users the choice to remain at home and recover more quickly, avoiding admission to hospital.

You will be part of a truly multi-professional team consisting of Consultant Practitioners, Physiotherapists, Occupational Therapists, Paramedics, Nurses, Apprentice Nurses, GPs and Administrators.

The team currently operates from five locations across the New Forest (Lymington, Totton, Romsey, Chandlers Ford & Fordingbridge) and each base has an established group of staff. The majority of time you will operate from your usual base, although there is some requirement to work flexibly from time to time across bases. Access to own vehicle for work is required. We provide a service to our patients 7 days a week between the hours of 08:30 and 18:00.

Main duties of the job

The post holder will be required to work autonomously. The foundation nature of the post means appropriate supervision from Consultant Practitioner is available to support the Foundation Consultant Practitioner to develop and work towards the competencies expected of a full Consultant Practitioner. These include;

Establish values based professional expert and evidence-based practice across pathways, services, organisations, and system working with individuals, families, carers, communities, and others.

Provide values-based leadership across the care pathway, services, and systems in complex and changing situations.

Develop staff potential, add to, and transform the workforce, and help people to learn, develop and improve (in and from practice) to promote excellence.

Develop a 'knowledge-rich and inquiry' culture across the service and system that contributes to research outputs and has a positive effect on development, quality, innovation, increasing capacity and capability, and making systems more effective.

Establishing expertise across the system and nationally by using consultancy approaches and opportunities that have maximum impact on practice, services, communities, and populations, and which add to and sustain workforce capacity and capability.

Qualifications

Essential
  • Appropriate professional nursing or allied health professional qualification with post registration experience of at least 5 years.
  • Professional UK registration e.g. NMC / HCPC
  • An Advanced Clinical Practice MSc accredited by the Centre for Advancing Practice
  • OR
  • Successful recognition with the Centre of Advancing Practice via the ePortfolio (Supported) Route to demonstrate educational equivalence that all the capabilities of advanced level practice at master's level (level 7) have been met.
  • Maintained a portfolio of ongoing evidence or work based learning.
  • Independent non-medical prescribing or pharmacology level 7 profession relevant qualification
  • Teaching and Assessing qualification or equivalent/ mentorship qualification.
  • Able to demonstrate a good command of the English Language both written and verbal
  • Computer Literate in both data entry and communicating via IT - emails and report
  • Experience of leadership in a healthcare setting.
  • Evidence of significant professional and managerial development, including a comprehensive and relevant CPD portfolio.
  • Evidence of working towards academic level 8 and/or through competency-based assessment of application of level 7 working, with scope to develop towards level 8 competencies.
