Job summary

This role welcomes applicants for AHP or NMC or qualified social work professionals.

The accountability within your role is in line with the Nursing and Midwifery council NMC and or the Health and Care professionals council HCPC, which says that you must work together for the safeguarding and wellbeing of the public.

Act as a Trusted Assessor for the Transfer of Care Hub Toch within the Acute hospital for packages of care, community beds across health and social care and third sector providers

Provide supervision, education, and training to the SPA and Community teams.

Act as an advocate for patients to ensure the highest levels of care are delivered.

Safeguard the delivery of high standards of care.

Deputise for the SPA Senior Clinicians to cover the oversight of the SPA and Trusted Assessment to community beds in their absence as and when required.

In conjunction with the senior clinicians provide effective clinical leadership.

Responsible for the delivery of high standards of nursing and therapy care to all patients.

Responsible for the clinical standards of care delivered within the service area.

Main duties of the job

To assess patients in the Acute hospital and to support flow from the Acute beds to the patient's homes with appropriate care or community bedded settings.

To communicate work with all areas of Transfer of Care Hub and the MDT to achieve the best outcome for patients

All actions are in line with NMC or HCPC codes and other statutory and organisational standards.

Educate and support staff. Identify staff development needs through the appraisal process

As a senior member of the team, act as a role model to all members of staff, students and visitors.

Support the team to manage & monitor clinical standards in line with the Trust policies through audit process.

To provide information, including data, performance and notification of incidents and accidents as required.

Initiate the investigation of concerns related to poor clinical practice or staff management.

Escalate all concerns when there is a concern over the care being delivered by an individual to the appropriate lead

Qualifications

Essential
  • Registered Clinician with current NMC or HCPC registration.
  • ENB 998 or other mentorship qualification.

Experience

Essential
  • Post qualification basic clinical experience.
