Job summary

An exciting opportunity has arisen for clinically focused, highly motivated PCN Clinical Pharmacists who wish to expand their experience by working part time or full timeon-sitein a General Practices within the Eccles area.

  • Independent Prescribing qualification is needed for this role.
  • Full time and Part time position available, 15 hours per week required for part time role
  • Must be local to Eccles area.

You will also be supported by a national network of pharmacists and technicians who are undertaking the same journey remotely and working on-site.

What we require -

  • Previous primary care experience
  • Experience with EMIS or System 1 is essential (Training can be provided)
  • Desirable to already be on CPPE Pathway or willing to partake
  • Able to commute to Eccles area
  • Independent prescribing qualification is required

Main duties of the job

In recognition of the important role clinical pharmacists can play in general practice we are seeking a professional pharmacist who can support the GP practice with general day to day workstreams using Emis or System 1, which may include the below:

  • Resolve medication-related issues
  • Helping to utilise skill mix
  • Improve patient outcomes
  • Ensure better access to healthcare
  • Support practice staff with prescription and medication queries
  • Medicines reconciliation
  • Provide expertise in clinical medicines advice
  • Providing clinical leadership on medicines optimisation and quality improvement

Job description

Job responsibilities

You will work as part of a multi-disciplinary team in a patient-facing role, working with colleagues to help resolve medication-related issues, helping to utilise skill mix, improve patient outcomes and ensure better access to healthcare.

You will support practice staff with prescription and medication queries. Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation, providing expertise in clinical medicines advice while addressing both public and social care needs of patients.

You will support with prescribing practices across the PCN, providing clinical leadership on medicines optimization and quality improvement.
