Job summary

As a vital member of our management team, the community Post COVID Service Lead will play a pivotal role in supporting our Haringey, Islington, and Enfield community teams in achieving key performance targets. They'll spearhead advanced clinical assessment and treatment for patients experiencing symptoms of Long COVID/Post-COVID-19 syndrome, employing a comprehensive biopsychosocial approach aimed at achieving patient goals.

Accountable for providing clinical mentorship, supervision, and educational support to our multidisciplinary team, the Lead will also drive ongoing service development within our department and extend support to wider community services.

With a focus on effective operational and clinical leadership, the Lead will take charge in fostering a culture of continuous quality improvement and professional growth. They'll ensure that patient care undergoes regular review and that service enhancements align with local, regional, and national directives.

As an expert practitioner in managing Long COVID/Post-COVID-19 syndrome, the Lead will operate autonomously, delivering evidence-based care, education, and support to patients and their families referred from UCLH Post Covid Clinic. This role presents a unique opportunity for professional development and skill enhancement in a culturally diverse geographical area.

Main duties of the job

  • As the Community Post Covid Service Lead, you will provide leadership, direction, and support for the Services. This includes offering clinical advice, managing a clinical caseload, ensuring policy compliance, and supporting staff for optimal patient outcomes.

Management/ Service DevelopmentTo lead and manage AHP/Nurse staff within the Community post Covid service, providing effective guidance, supervision and performance management including the education and management of students on practice placements.To contribute to the Trusts, Directorates and Teams Clinical Governance and Quality assurance programmes, setting and monitoring practice standards, jointly developing guidelines and protocols in liaison with multi-disciplinary colleagues.To inform senior management of changes in clinical practice which may influence service delivery, business planning and strategic reviews to support the delivery of the service.Establish service priorities and contribute to the development of the service including liaison with UCLH Post Covid Clinic and across NCL Community Teams, GPs, Pain Team and Long-term conditions teams.Be actively involved in service development through quality and clinical audit.To ensure own and others maintain accurate and up to date records of all professional interactions, decisions taken, related statistical information are kept and reported in accordance with Whittington Health and North Middlesex University Hospital NHS Trust policies.To participate as required in the recruitment selection and induction of other staff.To lead in the operational planning and implementation of quality improvement and service development within long Covid specific projects.To be responsible for optimising use of clinical staff and resources to ensure activity targets and trust and department efficiency and sustainability agendas are met.To lead on the development of strategies, to manage timely service provision and management of clinical priorities including when needs change.To attend regular meetings with those working in long Covid rehabilitation and those who interface with the pathway to create positive active partnerships in the best interests of those with long Covid.ClinicalThe post holder will practise in compliance with their code of professional conduct and within their scope of practice, being responsible and accountable for their decisions and actions at this level of practice.Use expertise and decision-making skills to inform clinical reasoning approaches when dealing with clients with complex presentation long term conditions, complex situations and be able to independently evaluate progress and adapt intervention as necessary.To have expert knowledge in the physical and mental health conditions that commonly are seen in those with long Covid diagnosis and to keep abreast of changes in this newly emerging clinical area.Carry out competent risk assessments when indicated for the psychological wellbeing of patients.Maintain accurate and up to date documentation for all of client caseload in interdisciplinary records.Be highly skilled in effective, sensitive, communication with patients with Long COVID, including discussion of sensitive information with clients and relevant parties. This will also include communication with patients where there is a language barrier.Be skilled in the management of clients in group settings such as symptom management programmes working closely with other members of the interdisciplinary team.Act as a resource for other members of the team in symptom management strategies relating to discipline specialty.Carrying out assessment and 1:1 treatment with clients with complex health, psychological and social needs requiring management of unpredictable situations, a problem-solving approach, empathy and negotiation skills.Ensuring that activity data is collected regularly and accurately, in line with Trust guidelines.Be responsible for developing written and multimedia resources in chronic fatigue for patients and the multidisciplinary team.Participate in and lead training in the management of Long COVID/Post-COVID 19 syndrome within the team, Long Term conditions teams, to GPs and other specialities/community organisations.Provide expert advice to educational establishments, workplaces and other community resources to promote clients successful (re) engagement in education / work / leisure in the community within appropriate professional capacity.Support, advise and provide education for patients and their carers as appropriate.Liaise with Whittington NHS Trust and North Middlesex University Hospital NHS Trust, external statutory and voluntary agencies to maximise engagement in the community. This may include providing advice / joint working on benefits and care plans.Allocate clinical workload appropriately to the team and prioritise effectively.Set a high personal standard of evidence-based practice and maintain up to date knowledge of clinical professional development in the field of occupational therapy and incorporate this appropriately into clinical practice. This involves regular attendance at internal and external training e.g. special interest groups in Long COVID.Ensure the clients management is goal directed, liaising closely with the multi-disciplinary team to enable effective discharge planning and act as a key worker when allocated.Participate fully in the interdisciplinary approach to rehabilitation working closely with all disciplines of the team and participating in team meetings and case reviews.Plan, organise and deliver specialist advice and teaching to colleagues, students and other members of the MDT.Work within Trust and Departmental policies and procedures.Always act in accordance with the Codes of Professional Conduct and Scope of Professional Practice

CommunicationUtilise reporting mechanisms including electronic reporting to ensure effective communication internally and externally to Whittington NHS Trust.Use effective interpersonal skills to encourage patients and carers to actively participate in supported self management as part of the therapy process.Maintain confidentiality consistent with Trust and Service policies.Act as an advocate on behalf of patients and carers by adopting a client centred approach to support meaningful plans for symptom management.To maintain and develop professional communication links across health and social care including representation at multidisciplinary collaborative forums and networks.To lead and participate in multi-disciplinary clinical discussions in the advancement of clinical care.To utilise advanced communication skills acting as a prime educator for students in the long Covid assessment clinic.ResearchEvaluate and audit own and others clinical practice, selecting and applying valid, reliable methods and then acting on the findings.Disseminate best practice research findings and quality improvement projects through appropriate media for example peer review research.Service development.Contribute and lead on quality improvement projects, audit and service development projects related to adult community services.To take an active part in continuing to develop new ways of working to increase equity of access to specialist intervention and support for clients with Long Covid across NCL and with partner organisations.
